[19a bottom] R’ Yose Hagelili: It is possible for two things to happen at the exact same time [ie: if a ewe’s two firstborn male twins’ heads both come out together, they must both be given to the Kohen].  Rabbis: Only one kid goes to a Kohen and one goes to the owner [after pasturing and developing a blemish], because it cannot be that they both exactly emerged simultaneously.
R’ Rosner quoting Rishonim: The reality is in G-d’s view, of course, one might be a millisecond before the other, but in our limited perspectives, we view them as simultaneous.

D-E-ep Thoughts [based on Minchas Chinuch]: For Mitzvos/Aveiros involving Time and Space (required places or amounts) even though we might not be exact [in Hashem’s eyes], it is “good enough” to say that we are.  We know we may not be perfect, but we must do what we are capable of [ie: if with our naked eyes there are no bugs on a lettuce, we do not need to examine it with a microscope]. Then, Halacha and Ultimate Reality gets re-defined by what our reality is (because Torah creates Existence).
